What planet are the D&D heroes from?

I was wondering if the D&D heroes have the same home planets as the rest of their race (i.e. humans from Earth). Or do they have a new home planet that is the setting for D&D? On a side note, what about the Dwarves squad and hero? Do they have the same home planet as the Dwarf from D&D? Sorry if this has been asked, but I didn't see it anywhere.

They are from the planet Toril which is the setting of the Forgotten Realms Campaign/Storyline (Elminster, Drizzt, et al).
